This is a really great poem! I always thought getting poetry from pages like this was really clever, and the poem you got out of it is also really nice! 
Though, when I count the syllables, I think the poem would be better off if the title was Memory, and then the rest of the poem is the actual poem. (if that's how you already have it, then you're good! XD )
I think the rhythm is also very nice and the idea is very interesting as well.
Not much else to say besides great job!
